PROGRAMS

Below is an overview of our programs, supportive services, and the intake process for individuals seeking assistance. 

INTAKE PROCESS

The goal of Home Again is to provide the path for homeless HIV/AIDS-infected individuals  to move to permanent housing and life as a productive citizen while experiencing positive health outcomes and reduced viral loads.

THE INTAKE PROCESS 

Potential residents or their representative must:

  • Submit Social Work and Physician Referrals Forms
  • Submit a Copy of Insurance Card and Picture ID
  • Submit A Negative TB screen (not older than 6 months )
  • Schedule an Intake Interview

CCFBA/HOME AGAIN RESIDENCE maintains a waiting list throughout the year and works to get clients housed as fast as possible.

SUPPORTIVE SERVICES

Housing Case Management: CCFBA’s housing case management is implemented following the five core principles of Housing First and Harm Reduction.

Medical Case Management: CCFBA’s Medical Case Manager will provide a range of client–centered services linking clients with health care, psychosocial, and other services that improve overall health.  

Psychosocial Counseling:  CCFBA provides a holistic continuum of care to serve clients with HIV/AIDS who are capable of self-care but requiring continuing psychosocial support services to ensure adherence to their care plans, reduce their viral loads, and remain in recovery.  We offer both individual and group sessions for HIV/AIDS-infected and-affected individuals and their families.

Substance Abuse Outpatient Counseling:  

OUR LICENSED PROFESSIONAL COUNSELORS  OFFER:    

• Psychosocial Counseling

• Substance Abuse Counseling 

FACILITY BASED/TRANSITIONAL HOUSING

HOME AGAIN RESIDENCE is a 24-hour facility based transitional housing facility.   CCFBA’s transitional housing options that are linked 

to supportive services provide a stable environment for a individual living with HIV/AIDS and homeless veterans.  

CCFBA’s Housing assistance services are provided for housing emergencies and are considered short term in nature and are for the purpose of maintaining an individual or family in a long-term, stable living situation. 

EDUCATIONAL PROGRAMS

 GED / WORK READINESS (HI-SET / GED Preparation Program)

The Adult Literacy Program provides students with every opportunity to build a strong foundation for their future as successful, contributing members of society.

CCFBA recognizes that many participants come to us without the credits needed to earn a traditional Louisiana high school diploma. For these individuals, this program offers an excellent alternative pathway.

Our HI-SET/GED Preparation Programequips students with the knowledge and skills needed to take and pass the GED exam and earn their General Educational Development diploma.

The program is designed for adults who have a valid reason for pursuing a GED. It provides comprehensive instruction in life skills, family relationship counseling, career and vocational development, and work readiness training, including résumé writing and job-seeking skills—all aimed at preparing participants to successfully earn their GED and transition into the workforce..
